If you are a new patient to Narromine Shire Family Health Centre please download and complete the below forms and drop them into our reception staff. Please note we only accept patient who live in Narromine or who live west of Narromine.

Patient Privacy: This practice protects your personal health information to ensure it is only available to authorised staff members for the intended purposes and to comply with the Privacy Act. To obtain a copy of our Privacy Statement or your medical records, please speak to our receptionists.

 

TRANSFERRING YOUR MEDICAL RECORDS

If you wish to have your medical records transferred from our practice to the practice you are currently attending or from your previous practice to ours please follow the steps below.

 

1. Fill out a transfer of records form from your current practice, detailing specific information you are needing transferred.

2. Fax the completed form to our surgery on 02 6889 4185

3. There will be an administration fee of $25 for the transfer of records from our practice to your new practice 

 

Please be advised in certain circumstances a transfer of records may incur a fee before they will be sent.
